                                   Welcome to the Vendor Registration/Payment Form

This form is used to select your level of sponsorship.  You can either pay on line via credit card 
(VISA, MasterCard, or American Express) and print your receipt, or, you can print an invoice to submit with your payment.

Please complete the form (fields marked with the * are required) as follows:

  1. Click in the check box or radio button next to each item you want to select.
  2. In the payment field pull-down box, click on each corresponding item that you selected in the first step. 
    To select more than one, hold down your Ctrl Key while clicking on the items you want to ensure the total amount  to be paid is correctly calculated and displayed.
  3. When ready, click on the Submit Form button and you will be go to the payment screen to complete the registration process.  You can either pay on line and print your receipt, or you can print an invoice to submit with your payment.
